You have probably arrived here by entering the Internet address provided in the letter sent to you
from the FSU School of Criminology. Within the letter you were provided a user name and password
to access the survey. After you've read this paragraph click on the link below that says “online survey form.”
Directions for completing the survey:
You will need to print a copy of the form, as some of the questions may need to be researched.
When you’ve compiled the answers on the print out, return to the Web survey and fill it out
online. Be sure to answer every question that has preformatted answers. Choose the answer that
best describes your agency. If you don't answer some of the required questions the survey will
warn you about missing answers.
