The following request for assistance form has been developed and used in schools as an effective communication document. Using a form such as this allows for:
- rapid communication between support staff and the teacher regarding action taken;
- assessment of targeted teacher staff development and/or resource needs; and
- documentation needed to determine alternative interventions or placements for the student.
