It is much easier to create a useful form if you have a model to work from. Start with a paper form or storyboard one. Think about how you would like to receive the data.

Hint: With OmniForm Pro you can create HTML forms by scanning in paper forms.

As soon as you choose your first form element, FrontPage creates the HTML and CGI code for a form. Or use the form page wizard to custom design a form.

FrontPage allows you to send your data to multiple places at once. These include email, directly to back end database applications, text files (for later data import), and can make Web pages to view data.
